Health insurance is automatically charged to all full time (enrolled for 12 credit hours or more) undergraduate student accounts at the time of registration. Other students may also choose to have health insurance added to their account. This is accomplished by completing the Health Insurance Request Form and submitting it to the Cashier's Office prior to the tenth day deadline as published in the Schedule of Classes each term. The form may be printed here (CLICK HERE) or from the printable forms section of Controller's Office Resources web site and the form is also available at the Cashier's Office.
The cost of the insurance is paid at the Cashiers' Office. Upon receipt of the insurance payment, the Cashier will distribute a health insurance brochure to the student.
Other students wishing to obtain student health insurance after the published deadline may apply directly with the insurance company. Further information may be obtained in Health Services.
Insurance cards will be mailed directly to the students by the insurance company.
The fee for health insurance may be waived for those students submitting a Health Insurance Waiver with proof of medical insurance at anytime prior to the tenth day deadline each semester.
